We have created one master intake form for your convenience so whether you choose to sit with one type of medicine or all three this will be the only form you will need to fill out.

The safety of all clients, as well as preserving the integrity of Kambo, is of the upmost importance
to us. As responsible Kambo Practitioners, it is integral that we understand where clients are
when it comes to their health and well-being and whether it is safe to administer Kambo to an individual.
To ensure the safety of everyone participating in a Kambo ceremony, it is extremely important for all
clients to answer the Kambo Session Intake Form with great care and honesty. This applies to listing
all medical conditions, medicines, supplements and any substances taken or being taken, as there may be
contraindications that could pose a risk to an individual’s well-being and safety.
IT IS IMPORTANT TO NOTE THAT NONE OF THE SERVICES OFFERED DURING THIS CEREMONY
CONSTITUTE ANY FORM OF MEDICAL PRACTICE. WE ARE NOT DOCTORS NOR ANY OTHER FORM OF MEDICAL PRACTITIONERS. WE DO NOT DIAGNOSE DISEASE, OFFER HEALTH ADVICE, OR PRESCRIBE MEDICINES. If you have or have had ANY of the following, you cannot safely take Kambo. It could be fatal, so please inform us immediately.- Serious heart problems
- Stroke
- Medication for low blood pressure
- Brain hemorrhage
- Blood clots/aneurysm
- Ehlers Danlos Syndrome
- Addison’s Disease
- Undergoing Chemotherapy or Radiotherapy for 4-6 weeks afterwards
- Taking immune suppressants for organ transplant
- Recovering from major surgery
- Pregnant or may be so
- Breastfeeding a child under 6 months
- Serious Mental Health Issues, excluding depression, anxiety, PTSD
- Anti-psychotics
- Methadone, Buprenorphine, Naltrexone, Krokodil (Desomorphine), Pink
- Lack the mental capacity to make decision to take kambo
- Under 18
The following are CAUTIONED so please inform us of details in the questionnaire:- Taking immune suppressants for autoimmune disorders
- Epilepsy
- Slimming, serotonin or sleeping supplements
- Active drug or alcohol addiction
- Long term or water fasting 7 days before or after Kambo
- Colonics, Enemas, liver flushes or any water based detox should be avoided 3 days before and after Kambo, as the loss of sodium could be dangerous.
- Sweat lodges, hot yoga and marathons should also be avoided 2-3 days before and after Kambo

San Pedro is a heart-opening medicine that is known as the grandfather medicine and is a cactus that is native to the Andes mountains. It has been used for healing for almost 3000 years. For many, a San Pedro journey offers deep insight into the self and the universe, giving one a greater sense of connection and spirituality. Mescaline is also known for fostering compassion and gratitude, while alleviating psychological disorders such as anxiety, depression, PTSD and addiction.
SAN PEDRO IS NOT SUITABLE FOR YOU:
- If you suffer from: schizophrenia, bipolar disorder, psychosis, borderline or certain other mental disorders.
- If you have or have had any of the following illnesses or conditions: severe kidney disease, unstable diabetes mellitus, epilepsy, severe neurological disorders, hypertension, cardiovascular diseases such as angina pectoris, strokes and CVAs, an existing or expected pheochromocytoma (a tumour of the adrenal glands), blood dyscrasias (abnormal cells), hyperthyroidism (overactive thyroid gland) and certain liver disorders.
- If you take antidepressants (you should stop taking them 4-8 weeks before your ceremony).
- If you take painkillers, sleeping pills or products for migraines and allergies (you should stop taking them 3 to 6 days before your ceremony).
- If you are pregnant or breastfeeding.
- If you want to combine alcohol or drugs like cocaine, amphetamine, MDMA, etc. with San Pedro.

Bufo Alvarius aka Toad, 5MEO DMT is the strongest entheogen/psychedelic in the world. It is known as the “God Molecule,” and can produce life changing results for the participant. That being said, this ceremony is not appropriate for persons with certain medical conditions or for persons using certain medications. If you have any doubt or concerns about whether you should participate, consult with Ashley.
Please be aware that the list of contraindicated substances includes, but is not limited to, SSRIs, antidepressants, anti psychotics. It is important to share truthfully and completely all information to ensure that you are fit for a safe ceremony with the Toad.
Pregnancy and breastfeeding are contraindicated. If a mother is in the final stages of breastfeeding, it is possible to pump, please speak to Ashley directly.
Failure to disclose any medical condition or medication can result in hazardous conditions for the participant.
It’s best practice to wait minimum of 7-10 days after any vaccinations you may have received, before sitting in ceremony with the Toad.
