If you are in need of more than one placard for your family.  Please complete this form so we can print and provide you with additional placards.  

Each family member/guest who picks up your child must have a placard in their front window in order for your students to be dismissed to them.  If you do not have a placard, you will be asked to go to the front office and show identification.
