Grandparent Afterschool Care Application
The grandparent and parent/legal guardian of a nonresident student requesting admission
due to after-school care provided by the grandparent are required to submit a Grandparent
After-School Care Application to the Superintendent or designee. Applications will be
reviewed to determine compliance with enrollment and residency requirements.

The Superintendent or designee is responsible for overseeing residency procedures and
ensuring that the reason for enrollment meets District guidelines.

Guidelines are as follows:

1. Grandparent resides within the Channelview Independent School District boundaries;
2. Grandparent is a biological parent of the student's parent/step-parent or legal guardian; and
3. Grandparent provides a substantial amount of after-school care for the person.
The District shall inform the parent or other person enrolling the child that presenting a
false document or false records in connection with enrollment is a criminal offense under

Penal Code 37.10. Education Code 25.001(d)



The grandparent providing after-school care for the child is required to complete this online form.

Upon completion, you will be contacted by the district to make arrangements to complete the Parent Affidavit



This information must be submitted for each nonresident person (hereafter called "Student") under
eighteen (18} years of age seeking District enrollment under TEC § 25.001 (b)(9) Grandparent in
District.

There are civil and criminal penalties for knowingly providing false information on a student
enrollment form, including reimbursement and criminal sanctions. See Texas Penal Code §3 7.1 0
and Texas Education Code §25.00l(h).
