2020 Little Stars Costume Order Form (Deadline to order is Sept 25th)

Please fill out 1 per class (if your child is in 2 or more classes please fill out one form for each class they plan to perform in).

Costumes are $60 each (includes tights). Your account will be charged on October 1st.

All classes ages 2-7 will participate exception of Hip Hop ages 6-9 and Musical Theater ages 7-10.

Your tuition must be current to have a costume ordered

No refunds on orders placed if you need to drop your class for any reason

Costumes will be handed out after the November 15th Tuition run and will only be handed out if your account is current.
