This form is to sign up for a survey being undertaken by the US Extremely Large Telescope Program's (US-ELTP) Research Inclusion Initiative (RII), which supports the research participation of the broadest US astronomical community by specifically addressing the concerns of researchers at small and under-resourced institutions that may be interested in doing research with 30m telescopes. To help the astronomical community develop collaborations, we are putting together a Research Inclusion Toolkit that will provide examples of good practice for research collaborations that embody the concepts of mutually beneficial partnerships. We're conducting this survey to better understand (1) How astronomers and astrophysicists experience cross-institutional collaborations; and (2) What unique pressures astronomers and astrophysicists at small or under-resourced institutions experience?
