I, the undersigned, have volunteered my services to Logan’s Heart & Smiles. I understand and acknowledge that the services I may be asked to provide include, among others, construction labor services. I acknowledge that I have the option at any time to refuse to volunteer my services. Should I choose, however, to volunteer my services to any specific project for Logan’s Heart & Smiles, I understand that I am doing so as a volunteer and will receive no compensation for my services, and that no other promises or inducements, written or verbal, have been made to me.
On behalf of the undersigned, I hereby release and forever discharge, indemnify and hold harmless Logan’s Heart & Smiles, together with its agents, employees, members, officers, owners, etc., from any and all claims of any kind and nature whatsoever arising through negligence, intentional acts or omissions, or in any other manner. This includes all past, present, or future actions, causes of action, demands, claims, damages, costs, attorneys’ fees, expenses and other compensation of any kind and nature that may arise out of any volunteer services provided by me to Logan’s Heart & Smiles or any project on which I am providing volunteer services.
I grant Logan’s Heart & Smiles the right to take and use photographs, video, audio and quotations of me in connection with Logan’s Heart & Smiles promotions, events and marketing. I authorize Logan’s Heart & Smiles, Its assigns and transferees to copyright, use and publish the same in print and/or electronically.

I agree that Logan’s Heart & Smiles may use such documentation of me with our without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising and web content without compensation.

This authorization can be revoked at any time by contacting us in writing. By typing your name below you are signing this document.
