2024 Spring/Summer Clothing & Toys Appointment Request
This form is for in-person APPOINTMENTS ONLY. 

Please fill out all the information requested on this form and click Submit. After all information is received, we will reply to your request with an appointment time and date confirmation in the order it was received.  Once you have received your appointment date and time, please keep your appointment so that all families can be served in a timely manner.  To contact the appointment desk, please email appt@fostercares.org or call 770-317-3749, 404-729-3374, or 770-280-7831. 

We ask for your patience as we process the lowest financially supported children first and then move on to those with Base per diems and SSI for the children. Please do not resubmit as it will delay processing. Only one designated shopper will be allowed to enter the facility to choose items for the children. Teens ages 9 and older may shop with their caregiver.  No biological parent is allowed in the facility.

Please note - the confirmation that we have received your appointment request is reflected at the end of this document at submission and not in a separate email.  Once we have reviewed and processed your completed request, you will receive a call with your appointment date and time.  We process over 2,000 requests at the beginning of each season and will reply as quickly as possible. Your request is very important to us and we thank you for your patience as we partner together to serve the children in your home!

Prom dress
: Although our volunteer numbers did not allow us to hold the 18th annual Prom-A-Palooza this year, we will have an assortment of prom dress in each size for Junior and Senior High School girls that would like to attend their high school prom this year. Please put the size, color and style you may be wanting, in the toy section so we can try to make sure your size has multiple selections. We can't promise to have your dream dress but will do our best. This option will only be available from March through the middle of May.

Did you want to submit a Mail Order Request instead? 
The Mail Order Request is a separate form for families who live more than a one-hour drive from the Roswell distribution center and are unable to make the drive for an appointment. To access the mail out form, please revisit the website and click on the Mail Order/Pick Up Request link at the bottom of the page. Due to shipping costs, mail out orders do not include infant equipment or large bags of toys, bikes or other large items. To receive all the extra items and have the opportunity to select your own clothing, we encourage scheduling an appointment to visit the distribution center instead. We also recommend appointments for teens who are more selective on their styles of clothing so they can select their own items.
