Seven Hills Ultimate Winter Season Tryout Registration Form 2023-2024

Please fill out this form if you would like to tryout for Seven Hills Ultimate Club. Players grade 6-8 may tryout for the Yellow and Green squads. Players grade 9-12 may tryout for the Purple, Blue, and Red squads.

The cost of tryouts is $100. Financial aid is available, select the amount that works best for you and your family. Thanks!

Key pieces of information:
  • Tryouts are Nov. 5th, 12th, and 19th. Attendance is expected at at least two of the three tryouts.
  • This program is ONE session per week on Sundays between December and Early March. 
  • There are ~10 sessions, attendance is expected at at least 8 of the 10. 
  • We may play inter-program scrimmages and those will take place during the Sunday practice session.
  • There may be 1-2 commitments outside of Sunday practices, they are not counted against your attendance requirements if your player can not make it.

In order to participate, all players must be USA Ultimate members. Prior to completing registration for Seven Hills tryouts, please follow the link below to sign up. If you already have a USA Ultimate membership, please find your membership ID number, and continue with registration.

Players may sign up for an affiliate membership which only covers our winter season OR may sign up for a full membership which covers future USA Ultimate events.


USA Ultimate offers grants for some players. Read below to determine if you may qualify. If so - indicate this in the registration document below and we will let USA Ultimate know to cover the fees for your account.

WAIVERS - 18 or Over Must Sign
The following questions constitute E-Waivers and must be filled out by an adult (18 or older) or by the parent or legal guardian of any minor child (under 18). You may email: sevenhillsultimate@gmail.com if you would prefer to print, sign and turn in the hard copy waivers.
Media Release:  I consent to interviews, photography (film or digital), recording (video and sound) of me/my player and their release, publication, exhibition, or reproduction to be used for public relations, news articles, telecasts, education, advertising, research, inclusion on the Seven Hills website or any Seven Hills social media, fundraising, or any other purpose determined by Seven Hills or its affiliates.  I release Seven Hills and its members, officers, agents and representatives from any liability in connection with the taking, recording, or publication using my/my player’s image, likeness or voice. *

Liability Waiver:  I understand the Ultimate Frisbee is a physical activity that involves considerable running and jumping and that collisions with other players and the ground can occur.  I recognize that Ultimate is a dangerous activity with inherent risks that can lead to serious injury, disability or death.  I expressly assume the risks related my/my player’s participation in Seven Hills activities.  I agree to release, hold harmless and indemnify Seven Hills and its members, employees, agents, contractors, volunteers and other representatives from all claims for any injury or damage resulting from any cause, including negligence, which arise out of my/my player’s participation in Seven Hills activities.  I accept full responsibility for all medical expenses and claims incurred as a result of my/my player’s participation in Seven Hills activities. *
