The above named contracts can be found in each section below. Both Students and Parents/Guardians must read each contract carefully, and then fill out the appropriate fields at the bottom of this form to indicate consent.  

PARENTS/GUARDIANS - IF YOU HAVE MORE THAN ONE CHILD IN THE DRAMA DEPARTMENT, PLEASE FILL OUT A SEPERATE FORM FOR EACH CHILD.

PLEASE SUBMIT THIS FORM PRIOR TO THE INITIAL AUDITION DATE OF YOUR FIRST AUDITION OF THE CURRENT THEATER SEASON (A theater season at STA starts in May of a calendar year and goes through April of the following calendar year - i.e., May of 2023 through April of 2024)

ONCE THIS AGREEMENT HAS BEEN MADE, IT IS VALID FOR THE REMAINDER OF THIS SCHOOL YEAR.
