If you wish to return to swim training, you must complete this health screen (one for each swimmer).

These are completely confidential and responses will be saved securely.

Fever
New, persistent, dry cough
Shortness of breath
Loss of taste or smell
Diarrhoea or vomiting
Muscle aches not related to sport/training

If you have had a confirmed COVID-19 infection or any symptoms in keeping with COVID-19 in the last 3 months, the Swim England guidance states that if you are now 7 days post-recovery with no symptoms then a gradual return to exercise is permissible. If you have persistent symptoms of breathless on exertion then you should consult your GP. *

The swimmer must not attend training if they have had exposure to anyone with confirmed or suspected COVID-19 in the last 2 weeks or if they have been advised to self-isolate. Swim England state the swimmer must not train until they have self-isolated for 14 days *

Swim England states if you have an underlying medical condition that makes you more susceptible to poor outcomes with COVID-19 then you should consider the increased risk and may wish to discuss this with your GP.
