* Age limit: 5 years old and above
* Payment: Zelle (jenaquaschool@gmail.com), Check (payable to: JENAQUA LLC) or Cash
* Refund Policy: There will be a 30% cancellation fee if cancelled before the swim program starts.  No refunds will be given once the program starts. Credit will be given only for medical reason with a doctor’s note.  
* All swimmers MUST complete the evaluation prior to processing the registration
* The swimmer's spot will NOT be secured until the payment is received.
* NO switching class time once the registration is completed.
** NO class credits/missed classes carry over to future usage.
** The group class is not a camp, and the tuition cannot be used for tax return.

It is expressly agreed that participation in all classes shall be undertaken voluntarily by the participant at their sole risk with knowledge of the dangers involved and the Jenaqua Swim School (Jenaqua LLC), its owners or employees shall not be liable for any costs, claims, injuries, deaths, damages, liabilities, or causes of actions whatsoever, to any person or property, arising out of or connected with the participation in any classes at the Jenaqua or the premises where the same are located including those arising from (i) acts of active or passive negligence on the part of the Jenaqua, its owners or employees (ii) the sudden and unforeseen malfunctioning of any equipment or machinery and (iii) participants slipping and/or falling while in the Jenaqua classes or on the premises, including adjacent sidewalks and parking areas, and participants do hereby expressly forever waive, release and discharge the Jenaqua, its owners, or employees from any and all such costs, claims, injuries, deaths, damages, liabilities or causes of action.

I hereby authorize Jenaqua Swim School to publish photographs taken of me or my children for use in Jenaqua’s print, online Facebook and video-based marketing materials, as well as other company publications. I further acknowledge that I will not receive financial compensation of any type associated with the taking or publication of these photographs or participation in company marketing materials or other company publications.
