In order for the boys to attend our trampoline park field trip, you as the parent must fill out a waiver provided by Defy Gravity Sacramento. If this form is not filled out, your child will not be allowed to enter into the facility so please click the link below and follow their instructions. I hereby give my permission for my child to participate in field trips and any other church related activities that require leaving the church grounds of MRCOG. I give my permission for my child to ride in any vehicle designated to transport MRCOG members to or from any church activity with the understanding that seatbelts will be used at all times. I understand that information regarding Kid Ministry activities will be posted on social media, in the church hallway, texted in our parent group text, etc. I understand that precautions will be taken to ensure the safety of all participants in any church activity. I do agree to assume all financial responsibility for above attendee in case of any injury incurred during MRCOG activities and will not hold MRCOG staff or chaperones responsible. I understand that if my child does not follow the discipline procedure during an event, I will be asked to come for my child and be with my child on future outings.
