Hello and thank you for your interest in participating in the 2023 TPLA Student Survey. Before you begin, we would like to tell you about the survey so that you can decide whether or not you would like to participate. Please read all of the information below carefully before proceeding.

• This survey is anonymous - we do not ask for your name (please do not put it anywhere on the survey). Because it is anonymous, we hope that you will be comfortable being completely honest when answering our questions.
  
• TPLA stands for Teen Prevention Leadership Academy. TPLA is part of the Union County Drug Free Coalition's efforts to empower Union County youth and adults to lead healthy, substance-free lives.
• This survey asks questions about experiences you or your friends may or may not have had regarding alcohol and or vape products.
• This survey will take approximately 3-4 minutes to complete
• Completing this survey is voluntary. You do not have to take the survey. Also, please feel free to skip any question that does not apply to you or that you are not comfortable answering.
