Reablement services for people living with dementia
This survey is addressed to people working in Black, Asian and minority ethnic led voluntary and community organisations which support people living with dementia from Black, Asian and minority ethnic backgrounds.

The survey is part of a project by Race Equality Foundation funded by the Health and Wellbeing Alliance looking at the accessibility and acceptability of services which support Black, Asian and minority ethnic people with dementia coming out of hospital and returning home.

The objective of the research is to better understand what organisations are doing that is helping people to live their lives and be more independent in their homes and communities. We will do this by asking questions about the types of services your organisation provides, what barriers people experience and examples of better practice.

Race Equality Foundation adheres to all data protection laws. Your identity will be protected. None of the answers you provide will be attributed to you or your organisation. The information you provide will be written up, anonymously, and key themes will inform recommendations to the Department of Health and Social Care and NHSE/I.
