This form is for exhibitors at ISHLT's 44th Annual Meeting and Scientific Sessions who wish to use the lead retrieval service with their exhibit booth. There is no additional cost for the service unless you need to rent QR scanners for scanning attendee badge QR codes or use the API. Even if you do not plan to rent a QR scanner device or use the API for your booth, you must complete this form if you would like to use this service with your own device.

*About:*
This service will allow you to log each attendee who visits the booth and have access to their information, including name, email address and organization/company name. You have the ability to add your own notes and use the optional ranking scale for each attendee. The information will be available to download as a CSV file. This service allows you to use your own device(s) without downloading a specialized app. *Lead retrieval data will not be stored on your device!*

*How To:*
Visit the webpage in your confirmation email and enter the passcode for your exhibit booth. From that page, you will be able to either scan QR codes and/or manually enter the attendee's ID (located on their badge). The attendees information will be displayed on the screen and you can press confirm to add them to your booth’s list of attendees or cancel to start again. Exhibit staff can use their own device(s), as most laptops, Android phones/tablets, iPhones, and iPads are compatible. The only requirement is that the device must have a camera and current web browser such as Google Chrome, Mozilla Firefox, Apple's Safari, or Microsoft Edge.

The URL and passcode for your exhibit booth will be provided before the conference starts. You can also get this at the Information Desk located at Registration.

If you rent QR scanners, they will be available on-site at the Information Desk located at Registration.
