This form is for campers attending Wild Child Day Camp in the summer of 2024. One form is required per camper per summer (not per week). If information will be different for different weeks, please indicate this in the "Other Comments" section at the end of the form, or email us at daycamp@childreach.on.ca - you will also be able to sign off on changes throughout the summer.
Confidentiality: The information provided on this form will only be viewed by or shared with the Camp Supervisor, Camp Director, and Camp Counsellors. Camp Counsellors will receive information about campers in their group, and information will be shared with all Camp Counsellors when required to ensure camper safety and a positive camper experience (e.g. allergies, asthma, supervision information, support needs). If required, Childreach Leadership (e.g. Program Manager or Executive Director) may also access camper information.
