You should use this form to book a place for your child on one of the school run clubs during the spring term 2022.

Payment for school run clubs must be made online through ParentPay at the time of booking. We strongly recommend that you pay for your club/s first on ParentPay and then complete this application form. Your child’s place will not be secure until you have made payment.

Please complete a separate form for each child you would like a book for.

Section 2 asks for information about your child, including parental and emergency contact telephone numbers and medical details. It also asks for information about who is authorised to collect your child from a club session.

Section 3 is the Extracurricular Clubs Home School Agreement. Please read it carefully and sign and date the declaration.

Section 4 is the booking form where you can select the club you would like to book.
