In consideration of participation with the Kids Summer Hoopfest, I hereby, for myself and members of my team, waive all and any rights or claims for damages I may have against the International Youth Education and Sports (iYes) Foundation, its Board of Directors, sponsors, referees or any other representatives for any and all injuries from whatever cause suffered by the players of my team while involved in any events organized by the iYes Foundation. I hereby give my consent to have a coach, team manager, emergency medical technician, nurse, medical treatment facility, and/or doctor of medicine or dentistry or associated personnel provide any member of my team with medical assistance and/or treatment and agree to be financially responsible for the cost of such assistance and/or treatment. I hereby authorize emergency transportation of any member of my team to a medical treatment facility should it to be warranted. I recognize the possibility of physical injury associated with basketball, and hereby release, discharge, and otherwise indemnify the iYes Foundation against any claim by or on behalf of the basketball players of my team named above as a result of my team's participation in Kids Summer Hoopfest and/or being transported to or from the same, which transportation I hereby authorize. *
