Player Code of Conduct:                                              As a player at DPA, I understand that I represent that the club whenever I attend a DPA event and that my actions have an impact on the image of the club.  I understand and agree to the following statements.  I understand and agree that a failure to abide by this code of conduct can result in a suspension or expulsion from DPA.     1.  I understand that the mission of DPA is to develop well-rounded soccer players and that games are used to further that development.  I understand that this mission is given a higher priority than winning games.  I will support this mission with my words, actions, and effort.     2.  I will learn the laws of the game and the applicable rules for each game or event.  I understand that the better I understand the laws and rules, the more enjoyable the game will be.     3.  I understand that my demeanor both on and off the field is a reflection of the club.  Poor sportsmanship and inappropriate behavior will not be tolerated.    4.  It is my responsibility to ensure that I arrive at all events at the required time and that I am prepared with all proper gear (i.e. all uniform pieces, shin guards, shoes, ball, and water bottle).     5.  I will play the game fairly and will not engage in actions intended to intentionally harm another player.     6.  I will never use threats, physical force, or obscene/abusive language toward any coach, player, spectator, or referee.      7. I will not speak or gesture to any referee in a negative manner, both during and after any game.  I will respect the decisions made by referees, even if I disagree with them.     8.  I will provide my coach with respect.  I will listen to my coach and follow through on the instructions that are given to me.     9.  I will never ridicule or yell at any player, including my teammates.      10.  I will not use alcohol or illegal drugs.      11.  I will address concerns with any coaches, club staff, or spectators in a calm and respectful manner.  All concerns related to a specific incident will be addressed no sooner than 24 hours after the time of the incident in order to allow for emotions to recede and ensure productive dialogue.      12.  I will remain with my team before, during, and after each game, until my coach dismisses me.  13.  I will maintain a positive attitude and work hard to develop my skills. *
Parent Code of Conduct:          As a parent/guardian with a child or children in DPA, I understand that I represent the club whenever I attend a DPA event and that my actions have an impact on the image of the club and development of its players.  I understand and agree to the following statements, and I take responsibility for ensuring that guests and/or friends with whom I am attending an event uphold the intent of this code of conduct.  I understand and agree that a failure to abide by this code of conduct can result in a suspension or expulsion from DPA for parents and/or players.       1.  I understand that the mission of DPA is to develop well-rounded soccer players and that games are used to further that development.  I understand that this mission is given a higher priority than winning games.  I will support this mission with my words and actions.     2. I will learn the laws of the game and the applicable rules for each game or event.  I understand that the better I understand the laws and rules, the more enjoyable the game will be.         3. I understand that my demeanor both on and off the field is a reflection of the club.  Poor sportsmanship and inappropriate behavior will not be tolerated.     4.  It is my responsibility to ensure that my child arrive at all events, including training and games, at the required time.      5.  I will never use threats, physical force, or obscene/abusive language toward any coach, player, spectator, or referee.       6.  I will not speak or gesture to any referee in a negative manner, both during and after any game.  I will respect the decisions made by each referee, even if I disagree with them.      7.  I will refrain from giving instructions to players during games, as I understand that my instructions may be either contrary to or distracting from the instructions of our coaches.  I will cheer good plays by both teams.     8.  I will never ridicule or yell at any player.  This includes my own child, other DPA players, and opposing players.     9. I will address concerns with any coaches, club staff, or board members in a calm and respectful manner.  All concerns related to a specific incident will be addressed no sooner than 24 hours after the time of the incident in order to allow for emotions to recede and ensure productive dialogue.     10.  I understand that during a game, I am not allowed onto the field of play, nor am I allowed to watch the game from behind the goal line.  This also pertains to instances in which a player is injured on the field.  In those instances, the referee will assess the injury and will either summon the coach or will resume play.  I will not enter the field of play unless authorized by the referee or coach.       11.  I will remain on the opposite side of the field as the teams before, during, and after each game. *
Waiver and Liability                                                          I agree that DPA, including its organizations, directors, offices, sponsors, employees, agents, successors, and assigns may, but have no duty to, provide me customary medical assistance, transportation, and emergency medical services.  I understand and agree that any time an individual is suspected of sustaining a traumatic brain injury or concussion, the individual should immediately seek the advice of a qualified and trained health care provider and should be monitored closely for the emergency of any signs and symptoms of an injury.  I hereby take the following actions for myself and my legal guardians, executors, administrators, heirs, next of kin, successors, and assigns:  a) I WAIVE, RELEASE AND DISCHARGE from claims or liabilities for death or personal injury or damages of any kind which arise out of or are related to my child’s participation with any and all staff or employees of Developmental Players Academy, all sponsors, and the trustees, officers, representatives, and agents of the above; b) I AGREE NOT TO SUE any of the persons or entities mentioned above for any of the claims or liabilities that I have waived, released, or discharged herein; and c) I INDEMNIFY AND HOLD HARMLESS the persons and entities mentioned above from any claims or liabilities assessed against them as a result of my actions.I HAVE READ THIS WAIVER, RELEASE OF LIABILITY, AND CONCUSSION POLICY AND I VERIFY THAT FULLY AGREE TO, UNDERSTAND, AND ACCEPT ALL OF ITS TERMS AND CONDITIONS. *
IN CONSIDERATION FOR PROGRAMS, FACILITIES AND ACTIVITIES of, Developmental Players Academy and The Behney Soccer Complex, 19904 Cornhusker Road, Gretna, NE 68028, or any purpose, including, but not limited to observation or use of facilities or equipment, or participation in any off-site program affiliated with Developmental Players Academy the undersigned, for himself or herself and any personal representatives, heirs, and next of kin, hereby acknowledges, agrees and represents that he or she has, immediately upon entering or participating inspected and carefully considered such premises and facilities or the affiliated program.  It is further warranted that such entry into The Behney Soccer Complex for observation or use of any facilities or equipment or participation in such affiliated program constitutes an acknowledgement that such premises and all facilities and equipment thereon and such affiliated program have been inspected and carefully considered and that the undersigned finds and accepts same as being safe and reasonably suited for the purpose of such observation, use or participation.  IN FURTHER CONSIDERATION OF BEING PERMITTED TO ENTER THE BEHNEY SOCCER COMPLEX FOR ANY PURPOSE INCLUDING, BUT NOT LIMITED TO OBSERVATION OR USE OF FACILITIES OR EQUIPMENT, OR PARTICIPATION IN ANY OFF-SITE PROGRAM AFFILIATED WITH DEVELOPMENTAL PLAYERS ACADEMY, THE UNDERSIGNED HEREBY AGREES TO THE FOLLOWING:  1. THE UNDERSIGNED HEREBY RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE The Behney’s, Developmental Players Academy and its owners, directors, officers, employees, and agents (hereinafter referred to as "releasees") from all liability to the undersigned, his personal representatives, assigns, heirs, and next of kin for any loss or damages, and any claim or demands therefore on account of injury to the person or property or resulting in death of the undersigned, whether caused by the negligence of the releasees or otherwise while the undersigned is in, upon, or about the premises or any facilities or equipment therein or participating in any affiliated program.  2. THE UNDERSIGNED HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS the releasees and each of them from any loss, liability, damage or cost they may incur due to the presence of the undersigned in, upon or about Developmental Players Academy premises or in any way observing or using any facilities or equipment of Developmental Players Academy or participating in any program affiliated with The Behney Soccer Complex, or Developmental Players Academy , whether caused by the negligence of the releasees or otherwise.  3. THE UNDERSIGNED HEREBY ASSUMES FULL RESPONSIBILITY FOR AND RISK OF BODILY INJURY, DEATH OR PROPERTY DAMAGE due to negligence of releasee or otherwise while in, about, or upon the premises of The Behney Soccer Complex and Developmental Players Academy and/or while using the premises or any facilities or equipment thereon or participating in any program affiliated with The Behney Soccer Complex or Developmental Players Academy.  The undersigned hereby acknowledges that the sport of soccer and training are inherently dangerous activities, and serious injury may result therefrom. I hereby agree to waive any and all claims, including but not limited to claims for injury or bodily harm, against all of the above parties, and their respective employees, volunteers, trainers, assistants, family members, spouses and agents in exchange for my/my child’s participation in the programs and activities mentioned above.THE UNDERSIGNED further expressly agrees that the foregoing RELEASE, WAIVER AND INDEMNITY AGREEMENT is intended to be as broad and inclusive as is permitted by the law of the State of Nebraska and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. THE UNDERSIGNED HAS READ AND VOLUNTARILY SIGNS THE RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT, and further agrees that no oral representations, statements or inducement apart from the foregoing written agreement have been made.   *
