This questionnaire will help us to begin our work together to build your custom designed birthstone piece.  We will contact you via email to follow up and start to mock up your piece together. Starting prices are listed, and may increase depending on chain selection/number of stones/length of final piece.

The best way to do this is to measure a piece of jewelry that you already own and love. If this is not an option, using a piece of string works as well.  Measure it so that it fits comfortably- we are looking for your desired length of the finished product, not the size of your wrist/neck.
