Please complete this survey by end of day Wednesday, Feb 3. The data will be used to help us come up with a plan to move forward.
Things we know:
1. Staff and student safety is our number one priority
2. We're moving to hybrid - date to be determined
3. Students will not change/rotate classes
4. Students will be in cohorts to minimize contact
5. We're anticipating about half of our students wanting hybrid
6. Virtual and hybrid students will be on the same schedule
7. Classes will be no more than 10 students at LEAST 6 feet apart

If BHS transitions to a hybrid model, do you plan on sending your student to in-person learning? (Students may not go back and forth between in-person learning and virtual learning without communication with administration first.)
Yes, I plan on sending my student for in-person learning.
No, I plan on keeping my student 100% virtual.

To send my student without a fever
To come pick up my student if my student comes down with a fever or COVID symptoms
To send my student with a mask and their chrome book for in-person learning
To pick up my student if they refuse to wear a mask or refuse to follow any COVID protocols (i.e. keeping 6 feet distance, staying in their cohort, wearing mask, following directional signs, etc. This is not an all incusive list)
If I am unable to pick up my student or to send someone to pick up my student and / or support the behavioralexpectations put into place by the school, my student will be returned to online learning only.
Do you require transportation for in-person learning?
