By providing your personal and banking information and submitting this form, you authorize the E.WE Foundation to initiate electronic funds transfers (ACH deposits) directly to your designated checking or savings account.

I understand that:

I may modify or cancel this authorization at any time by contacting the E.WE Foundation at admin@theewefoundation.org, and
this is a one-time deposit if I have been awarded a financial sponsorship through the E.WE Foundation's STRIPE Assistance program, and that any future awards will require a new authorization form, or
this is a recurring deposit if I am receiving a stipend through the E.WE Foundation's Internship program, and it will remain in effect until the end of my internship, or when I am no longer receiving a stipend.
