Johnny Bench Awards Nomination Form
Thank you for submitting your catcher for the Johnny Bench Awards.
Please note the follow criteria before submitting your catcher:

- The awards program honors the following 10 selections: National Men's Collegiate Baseball, National Women's Collegiate Softball, Ohio High School Baseball, Ohio High School Softball, Kentucky High School Baseball, Kentucky High School Softball, Indiana High School Baseball, Indiana High School Softball, West Virginia High School Baseball, West Virginia High School Softball.

- Candidates are not restricted by class year or level of competition.

- Athletes must have played more than 50% of their games at the catcher position.

- Complete offensive and defensive statistics will be requests at the end of the season and are required for award consideration.

- In this survey you will be asked if this catcher was nominated to the All-State team. Such accolades are not required but allow us to better track catchers' information throughout their school career.
