Alpha Delta Inventory Check Out/In Form
Hi everyone! This is an inventory check out/in form I'll be using for the Fall 2023 semester to keep track of what goes in and out of the storage unit. This form will serve as an official record for this semester, meaning no items will be considered checked out or in until it is formally submitted through this form.

A couple of things to note that will be enforced this semester:

- For any items that are being checked out, the earliest I ask for this form to be submitted is at least a week before it's needed. The latest I'll accept is 3 days before it's needed. If the request is submitted less than 3 days before it's needed, there is a chance the item will not make it in time.

- A request given by word of mouth is not considered official. In the event this happens, I will direct you to this form.

- Anything given out before this form became available to the chapter (08/21/23) is considered official and approved. Please fill out the form for any items currently checked out.

I know this is a little lengthy, but it's really just to keep inventory as updated as possible and to ensure items will be at the event. Thank you for understanding.
