Booster Club Meeting Form

We need at least 5 parents to establish the Clark MS Choir Booster club. For the first meeting we will:
- Vote to form a booster club
- Vote to adopt the bylaws
- Vote to adopt the addendum to the bylaws (which include membership fee information - my thought would be around $50)
- Vote on officers (we should generally have this agreed upon prior to our meeting)
- Vote to obtain a tax ID
- Vote to either establish your entity on its own which will take a significant upfront investment with the IRS, or vote to establish your relationship with an umbrella organization.
-Adjourn meeting

Once we have a record of the meeting notes, the treasurer can begin establishing the tax ID and connection with parent organization (such as Parent Booster USA) or directly with the IRS, depending on what we decide.
