Kindergarten 2023-24
Hello!
It's time to sign up for Kindergarten!  Children must be 5 by September 30.  Screening will take place Thursday, April 13 and Friday, April 14.  This is by appointment and will be 60-75 minutes in length.  A custodial parent/guardian must attend with the child (both parents are welcome).  We will have computers available as parent(s)/guardian(s) will be completing the enrollment process during Kindergarten Screening.  Please come prepared with:

1.  certified birth certificate
2.  vaccination (shot) record
3.  custody paperwork (if applicable)
4.  proof of residency (2 documents)

We will be filling time slots on a first come, first serve basis. You will get an email from Karen Kuney confirming the date and time that you choose in this form for your child's appointment.   If you have any questions please call 419-924-2365 Ext. 2350.

After parking, come to the Elementary Entrance and ring the bell to enter the office.  Please come in time to be ready for the appointment as we will be starting as close to the scheduled time as possible but do not come more than 15 minutes prior to the start as we have other sessions that will be finishing up.
