Production Fee Agreement: I, the legal guardian of the participant named above, understand that Scripps Theatre Arts requires a production fee to be paid for participation in stage productions. The production fee for Shrek is $300 and is due by the first rehearsal. I agree to pay this fee via: *
Behavior Agreement: I, the legal guardian of the student named above, agree that my child will adhere to all behavior expectations (wearing face coverings if they are again deemed to be required at rehearsal, practicing respectful behavior, paying attention when asked) and that I may be called to pick up my child if he/she/they struggle to follow directions. I have explained these rules to my child. *
COVID-19 release: I, the legal guardian of the student named above, understand that Scripps Theatre Arts cannot guarantee that its participants or others in attendance will not become infected with the virus that causes COVID-19 and that Scripps Theatre Arts, along with anyone associated with Scripps Theatre Arts, cannot be held liable for such instances. *
COVID-19 Agreement: I, the legal guardian of the participant named above, agree that if my child is exhibiting any symptoms known to be associated with COVID-19 (including but not limited to fever, cough, difficulty breathing or shortness of breath) that I will not bring my child to rehearsal or performances that day. If my child begins experiencing any symptoms during rehearsal or performances, I agree that I (and/or the emergency contacts listed above) will be called to pick my child up immediately. *
