I understand that it is my responsibility to review the Enrollment Agreement, Program Closing Procedures, Schedule Change/Termination Form, Program Reminders and Summary Sheet which I will receive via email in August, 2023. I agree to all the terms and policies of this program. I understand that I am responsible for updating any information, in writing, and submitting it to the Program Office immediately should anything change after my initial registration.

I acknowledge that submitting this form does not guarantee my spot(s) and that once payment is received I will receive an email confirmation to let me know my registration is complete.

I do hereby waive, release and hold harmless the North Babylon Almost Home Program, their employees, volunteers, and instructors for any problems, damages, injuries or losses that may be suffered by my child while participating in this program.
