Survey Overview
This study is a part of a research exercise conducted by Winnie Lao for the INF2191 User Interface Design class for the Faculty of Information. The purpose of this study is to understand the restaurant ordering habits of individuals to identify pain points that people experience while ordering food. Findings from this study will be used to develop a product to address the needs gaps of people ordering food.

Participants will complete an anonymous questionnaire. All questions are optional. Participation is voluntary and participants have the right to withdraw at any part of the study without penalty. Information collected can be removed from results upon request.

Informed Consent
As an adult aged 18 years or older, I hereby consent to participate in a study conducted by Winnie Lao for INF2191, for the purpose of exploring food ordering habits. I understand that:

- Instead of monetary compensation, I get to participate in research, express issues and contribute to this study.
- I may express concerns, ask to pause, skip an activity, or withdraw at any time without explanation or penalty.
- My data will be anonymized and my identity or any identifying information will not be associated with it.
