Thank you for taking the time to share your experience in our Diversity, Equity, and Inclusion Survey. We can’t improve what we don’t measure, and that begins with providing a safe space to hear your sentiments and concerns.

The survey consists of 20 questions distributed among the following categories:
Your Identities, Diversity, Inclusion, Equity, Racial Justice, Harassment and Discrimination, and Looking Forward.

Some of the questions are on a 1-5 disagree/agree scale so we can quantify how our community feels, and set measurable improvement goals. We encourage you to tell us more about WHY you feel that way in the text box after each of those questions.

CONFIDENTIALITY:  This survey is anonymous, and if you choose to tell us who you are, that information will be kept confidential. If you are logged in to a Google account, at the top of the survey you should see a crossed-out eye with "your-username@gmail.com (not shared)." To remove all doubt, we recommend filling out this Survey in a private browser session.

REPORTING:  Any responses will not be individualized. We will share the data we gather from this survey in an aggregated manner: we will communicate collective numbers in order to prevent identifying individuals, while allowing us to advocate for different groups of people in our community.
