For each $100 in ShopRite and/or Stop & Shop card purchases, $5 will be donated to FONS to support our choir and performing arts students and programs. Gifts cards may be purchased in multiples of $100.

Payment is due at time of card pick-up. Cash or check* payable to FONS Student Fundraising.

Once you complete this form, we will be in touch to schedule pick-up.
