ACKNOWLEDGEMENT OF GUARDIANSHIP & RISK:  
I hereby acknowledge that I am the parent/legal guardian of the archer named on this form, who is a minor under the age of eighteen (“Minor Child”). In consideration for the benefits to be derived from My Minor Child’s participation in archery, I, on behalf of myself and My Minor Child, hereby acknowledge the following:
(1) I am aware that all archery activities involve risk;
(2) I am aware that archery activities will be a dangerous activity involving MANY RISKS OF INJURY; and
(3) I UNDERSTAND THAT THE DANGERS AND RISKS OF ARCHERY ACTIVITIES INCLUDE, BUT ARE NOT LIMITED TO DEATH, SERIOUS INJURIES, WHICH MAY RESULT IN COMPLETE OR PARTIAL PARALYSIS, BRAIN DAMAGE, SERIOUS INJURY TO VIRTUALLY ALL INTERNAL ORGANS, BONES, JOINTS, LIGAMENTS, MUSCLES, TENDONS, AND OTHER ASPECTS OF THE MUSCULAR SKELETAL SYSTEM AND SERIOUS INJURY OR IMPAIRMENT TO OTHER ASPECTS OF MY MINOR CHILD’S BODY, GENERAL HEALTH, AND WELL-BEING. On behalf of myself and my Minor Child, I further understand and acknowledge that the dangers and risks of archery activities may result not only in injury, but serious impairment of My Minor Child’s future abilities to earn a living, to engage in other business, social, and recreational activities, and generally to enjoy life. Additionally, I realize that archery injuries can be catastrophic for those without proper medical coverage.  *
RULES AND REGULATIONS: 
Because of the danger of participating in archery and related archery events, both on my own behalf and on behalf of my Minor Child, I acknowledge and understand the importance of following rules and regulations established by Just AIM, LLC, their coaches, parent volunteers, and affiliate organizations, and hereby agree to be subject to, bound by, and observe all applicable provisions of the bylaws, rules, codes, regulations, policies and procedures, and any other applicable governing bodies associated with archery, practices, tournaments, and related events.  My minor child hereby agrees to obey such rules, regulations, and instructions. *
STATEMENT OF HEALTH AND SAFETY: 
I understand that archery is a physically demanding sport and hereby certify and further acknowledge that my Minor Child is in good physical condition and do not know of any mental or physical condition or reason that would preclude my Minor Child's ability to safely participate in archery Events & Activities.  I further agree that my Minor Child is responsible for their own health and safety at all archery Events & Activities, and should immediately discontinue participation in such activities if there is a health or safety concern.

ASSUMPTION OF RISK
My Minor Child is VOLUNTARILY PARTICIPATING in the aforementioned archery activities, and on behalf of myself and my Minor Child,  I HEREBY RECOGNIZE AND ASSUME ALL THE RISKS ASSOCIATED WITH MY MINOR CHILD PARTICIPATING IN ARCHERY AND RELATED ACTIVITIES. I am aware of the the aforementioned risks associated with participating in archery events and activities and UNDERSTAND THAT THESE INJURIES OR OUTCOMES MAY ARISE FROM MY OWN OR OTHERS' NEGLIGENCE, CONDITIONS RELATED TO TRAVEL TO AND FROM THE ACTIVITY, OR FROM CONDITIONS AT THE ACTIVITY LOCATION(S). NONETHELESS, I ASSUME ALL RELATED RISKS, BOTH KNOWN AND UNKNOWN TO ME, OF MY PARTICIPATION IN THIS ARCHERY ACTIVIES, EVENTS, & TOURNAMENTS.

WAIVER AND RELEASE OF LIABILTY, HOLD HARMLESS AND INDEMNITY:
On behalf of myself and my Minor Child, I AGREE TO DEFEND, INDEMNIFY AND HOLD HARMLESS JUST AIM LLC, IT’S, AGENT REPRESENTATIVES, COACHES AND VOLUNTEERS FROM ANY AND ALL OBLIGATIONS, LIABILITIES, CLAIMS, DEMANDS, COSTS, AND EXPENSES, INCLUDING ATTORNEY’S FEES, OR DEMANDS OF ANY KIND OF NATURE WHATSOEVER WHICH MAY ARISE OR IN CONNECTION WITH MY MINOR CHILD’S PARTICIPATION IN ANY ACTIVITIES RELATED TO ARCHERY, INCLUDING INJURIES SUSTAINED AS A RESULT OF THE SOLE, JOINT, OR CONCURRENT NEGLIGENCE, NEGLIGENCE PER SE, STATUTORY FAULT OR STRICT LIABILITY OF JUST AIM LLC, THEIR COACHES, VOULENTEERS, & AGENT REPRESENTATIVES.  The terms hereof serve as a release and assumption of risk for myself and on behalf of my Minor Child, as well as our respective heirs, estates, executors, administrators, and assignees.

COMPLETE AGREEMENT AND SEVERABILITY CLAUSE:
his agreement represents the complete understanding between the parties regarding these issues and no oral representations, statements or inducements have been made apart from this Agreement.  If any provision of this Agreement is held to be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from this Agreement and shall not affect the validity and enforceability of any remaining provision.  In signing this agreement, I acknowledge and represent that I have read and understand it; that I am the parent or legal guardian of the named child; that I sign it voluntarily and for full and adequate consideration, fully intending to bind myself and the named child by the same; and that I am at least eighteen (18) years of age and fully competent.  I HAVE READ THIS AGREEMENT, UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND AGREE INDIVIDUALLY AND ON BEHALF OF MY MINOR CHILD TO BE BOUND BY IT. *
