RMS Yearly Permissions and Notices 2021-2022
Every year, we ask families to grant permission for their students to the school and district for certain events, activities and programs.  In an attempt to streamline and expedite gathering information and in lieu of filling out and signing forms to be returned with students or emailed, we invite you instead to complete this Google form indicating your preferences.  We will reference this information over the course of the year by updating it in Synergy and in your student's cumulative file.  Thanks in advance for completing it.  Links to each PDF form can be found on the 4J website at https://www.4j.lane.edu/instruction/enrollment/student-enrollment/ or below in the relevant section.
