Emerging Leaders Mentor Program- Mentor Questionnaire
(Note: Must be a current member of AL!VE and maintain membership throughout the length of the program in order to participate)

Thank you for volunteering your time to serve as a mentor in the AL!VE Mentor Program. This program is designed as an opportunity for volunteer engagement professionals to offer guidance, support and information to fellow professionals that want to sharpen competencies that will assist them in their own professional growth and development.

Mentees will be matched with mentors that can best assist the mentee in the competencies they are most interested in developing. AL!VE will make every effort to find a match for everyone. However, not all members completing the questionnaire may be selected as there may be more mentors than mentees available. Those selected will be based on what matches are most compatible.

As a mentor, completing this questionnaire will assist us in matching you with the best possible mentee. Please think about your entire professional career and experience when completing this questionnaire so we can learn the most about you!

Mentors and mentees will attend orientation in order to find a suitable mentor match. Once you attend orientation, mentors and mentees will be notified of their match by the following week.

IMPORTANT: We will contact you before we open the application process for mentees to share the program's timeline and confirm you are still interested in participating.  This questionnaire will be made public to those who want to participate in the program so they can provide the preferences for a match. By submitting this questionnaire, you are giving us permission to share this with potential mentees.

Mentor Readiness Checklist
Can you say Yes to all the following questions?

1. Are you willing to commit to a virtual meeting monthly with your mentee for six months as well as attend orientation, a midpoint check in, and final meeting with all participants?

2. Do you have experience, knowledge and competencies (e.g., fundamental volunteer management skills or leadership skills) to serve in a mentor role?

3. Are you willing to commit the time, energy and resolve needed for ensuring a successful mentoring relationship, being responsive to your mentee's calls or emails?

4. Are you ready to learn, listen and ask questions of your mentee in order to provide them constructive feedback, even if it may appear to be a difficult conversation?

5. Are you ready to develop an open, honest, and transparent relationship committed to achieving your mentee's development goals?

6. Are you willing to complete the program evaluation to provide your candid feedback about this program, its successes and opportunities for enhancement?
