Parent agreement, field trip and medical consent *

I acknowledge that Allegro Child Care Centre Ltd., its agents and its employees, shall not assume any responsibility in the event of sickness or injury to my child while in attendance at or in transit to or from any field trip or excursion under proper supervision by Allegro Child Care Centre Ltd., except for injuries or damage resulting from negligence of Allegro Child Care Centre Ltd.

I herby authorize Allegro Child Care Centre Ltd and its employees to take my child on any appropriate field trips/outings.

I hereby authorise Allegro Child Care Centre Ltd., in the case of medical emergency to arrange medical care for my child by a medical professional and/or at a local hospital, and I further consent to any necessary medical treatment of such child. I give permission to the staff of Allegro Child Care Centre Ltd to apply sunscreen, as needed, to my child on a daily basis.

I also agree that I have received and will adhere to the policies as stated in the Welcome and Pandemic Manual.
