Fall 2022 Mentoring Relationship Covenant
This form is to be filled out with both the mentor and mentee during your first meeting together. Only ONE form needs to be turned in by the mentor, but you will work through it together, dialoguing and recording responses.

This form serves two purposes: 1) It clearly lays out the aspects of this covenant mentoring relationship within the Faculty/Staff Mentoring Program, and 2) it allows the two of you space to brainstorm, process, and clearly communicate your OWN expectations to each other.

What are you REALLY looking for in this relationship? What might hinder that hope? What might make it possible? Be as honest and thorough as possible.

Once you complete this form together, a copy will be sent to the email collected for your reference. We invite you to use this form to continually "center" your relationship around the goals and expectations originally determined. If you need to re-write your expectations, please fill out the form again at any time!

We commit to fill out the end-of-semester evaluation in early December (to be sent then) to update the Mentoring Office about the status of our mentoring relationship and our experience of the Discipleship Mentoring Program. *
