DFX REGISTRATION 2023-2024
This form MUST be fully completed in order for your child to participate in class.  Please read the questions carefully before answering - the information requested is important to ensure that we have the correct information for attendance and billing purposes.  You are responsible for knowing the policies that you are agreeing to, so please ensure that you read them carefully.  If you have any questions, please direct them to Lisa @ dance.fx.lisa@gmail.com                  CLASSES BEGIN TUESDAY, SEPTEMBER 5TH.

There is a one time registration fee of $30 per dancer or $45 per family.  Returning dancers who register PRIOR to THURSDAY, JUNE 15th, your registration will be half price.  

***We are asking that all dancers bring their own refillable water bottle next season, to eliminate the waste from disposable cups & bottles currently being used.

There is a one time, non-refundable, registration fee of $30 per single dancer, or $45 for a family.   *
Tuition is due the first week of every month.  If tuition is not received before the 10th of the month, you will be charged a late fee of $10.  At that point, late fees will be considered part of your tuition and must be paid in full in order for your dancer to participate in class.    ** If circumstances arise which prevent you from paying tuition in full and on time, you will contact Lisa to make arrangements, PRIOR TO THE 10TH OF THE MONTH.  If you choose to pay the entire season in full PRIOR to the end of October, you will receive one month free. If you opt to drop classes, you will inform Lisa prior to the 1st of the month or you will be responsible for tuition for that month. *
All payments (tuition and other fees) must be submitted to Lock Box located on wall near reception desk, unless paying by Venmo.  All payments should be labeled with DANCER name and description of fee.  There are envelopes provided for cash payments.  Receipts for cash payments will be ready for pickup at next class.  ***We WILL NOT provide receipts for credit card or checks.***  Your bank/card statement is your receipt. *
If you fail to pay tuition for 2 months, your dancer will not be allowed to participate in class until payment is made. *
Dancers should not miss more than 6 classes.  If your dancer misses more than 6 classes, the teacher reserves the right to prevent participation in recital.  If a dancer is unable to participate due to medical or non-payment reasons, when physically able, dancer should still attend class in order to observe the lessons and choreography.  Please understand that absence from a dance class affects not only your dancer, but every dancer in the class.   *
Dancers must arrive and be prepared to dance on time.  If your dancer misses the stretching portion of class, they will not be permitted to participate in class as failing to stretch can lead to injury. *
Dance FX studio opens 15 minutes prior to the first class and closes 15 minutes after the last class.  NO STUDENTS SHOULD BE DROPPED OFF BEFORE STUDIO OPENS and ALL STUDENTS SHOULD BE PICKED UP NO LATER THAN 15 MINUTES AFTER THEIR LAST CLASS.   *
Dancers must wear appropriate dance-wear to each class.  This includes dance shoes.  Each teacher determines what is appropriate for their classes.  You will be informed of required attire in confirmation email.  If dancer fails to meet these requirements, she/he will not be permitted to participate in class. *
The first month of classes is used by the Dance FX Teachers to assess the dancers and to determine appropriate levels.  If you believe your dancer is not in the correct class, please bring your concerns to your teacher within this first month.  Choreography for the annual recital begins in October, therefore, changes must be made in September. *
Dance FX provides a dressing room for female dancers and a restroom for male dancers in which they can change.  Please ensure that no person of the opposite gender enters designated dressing/rest-rooms, as we do have dancers who do FULL changes and require privacy.   *** All personal belongings should be kept in the dressing rooms (including cell phones).    Dancers may bring meals, snacks and drinks, but they are responsible for cleaning up after themselves.  Dance FX also provides recycling...please pay attention and place your items in the correct recycling bins! **SUBJECT TO CHANGE DUE TO COVID-19.   *
Students must keep their cell phones in the dressing rooms or at home.  They shall NOT be kept on the counter or anywhere that they can disrupt class.  If they are, Dance FX staff will hold them in the office until class is over. *
Teachers are scheduled to teach classes with very few breaks.  They come from other jobs to teach our dancers.  If you need to talk to a teacher or the Director, please email dance.fx.lisa@gmail.com  to request an appointment. Lisa will be the ONLY staff to manage tuition.  All Dance FX Staff are more than happy to talk to you and address any concerns you may have, but please allow them to do it at a time that does not take away from the instruction of our dancers. *
