Your cooperation will assist church leaders in their efforts to provide a secure environment for children or youth who participate in our programs and use our facilities. Your responses will remain confidential, although there may be circumstances where such information may be provided to individuals working with our ministry and to other individuals in order to evaluate your application and/or to comply with applicable legal requirements.

The information contained in this application is correct to the best of my knowledge. I authorize any references listed in this application to give River City Church any information (including opinions) that they may have regarding my character and fitness for ministry with children. In consideration of the receipt and evaluation of this application by River City Church, I hereby release any individual, church, youth organization, charity, employer, reference, or any person or organization, including record custodians, both collectively and individually, from any an all liability for damages of whatever kind or nature which may result at any time to me, my heirs, or family on account of compliance or attempt to comply with this authorization.
