Hello! My name is Taylor Addis and I am currently a junior at Westfield High School. I understand the peril that the pandemic has caused and how difficult computers/getting an available vaccination appointment can be. That is why I have made this easy and quick form! Simply fill out some basic information and when available appointments are announced, myself and other recruits will make it our top priority to sign you up!

You should (not all sites send confirmations) receive a notification from the site of your scheduled appointment with a follow up email from gen2genhelpers.com. Please check your emails daily since many appointments are booked at night and are scheduled for the next day.

Please do not sign anyone up without telling them first and make sure you let us know ASAP if you scheduled an appointment on your own. We are receiving  a lot of messages telling us they have received a vaccine after we send out notifications for appointments. Thank you!!
