(免責同意書）私は自らとそして家族のJWCの活動への参加は私個人の任意であることを認め、下に署名します。私はこの文章に於いて家族のJWCの 活動やプログラムに参加する許可を与えます。私はここにおいて、JWC及びJWCマネジメントスタッフ、JWCボランティア、JWCメンバー、JWCイベント参加者、および活動場所提供機関や材料より JWC活動中で発生した事故、病気やけがなどの責任は一切問わないことに同意します 。著名することでJWCに関わる個人や機関に対する起訴や責任追究する権利が除外されることを意味します。 自身と家族の安全に対する最終的な責任は私にあることをここにおいて認めます。
Hold Harmless Agreement I understand and undersigned that my participation and the participation of any members of my family in JWC activities or programs is completely voluntary. I hereby give permission for myself and my family to join JWC activities. I and My family shall hold harmless JWC, its past and present management staffs, volunteers, members, or participants and or the providers of any activity or program location and/or materials from any liability and or responsibility for any accident, illness or injury that occurs during or as a result of any function or program. My signature below has the effect of precluding any lawsuit you might file, or from otherwise seeking damages against the people and/or organizations listed above. I accept that the final responsibility for my safety and that of my family rests with me.
