23-24 Absence Notification Form BHMS

This form is for notification of absences.  Completing this form does not automatically excuse absences. The determination will be made by the Attendance Office in accordance with the City Schools of Decatur attendance policy (page 33).  All request must be submitted by the parent/guardian on the day of the absence, NOT before. Students are not allowed at any time to complete this form for themselves, those entries will not be accepted. 

If a student is listed in Infinite Campus with 10 or more excused or unexcused absences a medical note is required to be excused for future dates.
