Livermore Little League: I need a background check to volunteer!
This form is only to be filled out if you have not yet submitted your background check, AND you have not received an email from JDP yet.  Please look in your email, it will come from
Livermore Little League <automation@instascreen.net>

Once this is filled out, we will capture the form submissions each night, and upload to JDP and then you will receive your email.

Most all have already been uploaded to JDP, so please double check that you have not yet received this so that it's not duplicated.
JDP is the Little League approved background check company, you will provide them your information via their secure website. (This is much safer than previous years where we capture a printed form with all your personal info).

Once submitted, it can take 24 HOURS to receive your JDP background check email.
