NOTE:  THIS WAIVER AND RELEASE MUST BE ACKNOWLEDGED BY ALL PARTICIPANTS. IT RELEASES AMERICA’S BIG SISTERS, DR. TRAGIL WADE-JOHNSON, AND THEIR OFFICERS, EMPLOYEES AND AGENTS FROM LIABILITY.

In consideration of Participant’s entry and participation of Participant’s own free will in Dr. Tragil Wade-Johnson, Tragil Wade Inc., and America’s Big Sisters Foundation (ABSF) and Wayne State University negligence of the parties herein.  Participant is fully aware of the risks in participating in an Event of this nature and fully understands that Participant is forever giving up in advance any right to sue or make claims against the parties.  Participant is releasing the parties if Participant suffers such injuries and damages even though Participant does not know what or how extensive those damages might be and is voluntarily assuming the risks of such injuries and damages unless caused by the gross negligence of the parties herein.   

The undersigned also acknowledges the fact that the Event may be taped and photographed.  The undersigned hereby grants to Dr. Tragil Wade-Johnson and America’s Big Sisters (ABSF) and ABSF may grant to others the exclusive worldwide rights in all media (including over-the-air, cable, satellite, internet and other means of distribution, transmission, exhibition and reception, or combination thereof, now and hereafter devised) to televise the Event on a taped-delay basis in perpetuity.  By signing below, I also hereby authorize ABSF to use my image, voice and/or performance for publicity purposes to promote Dr. Tragil Wade-Johnson and America’s Big Sisters (ABSF) and their cause.  I agree that there will be no compensation whatsoever for this participation or for the use of resulting materials by Dr. Tragil Wade-Johnson and ABSF.  I understand that no personal history information will be revealed by ABSF.  The undersigned agrees that ABSF shall have and may grant to others the right to use Participant’s name and likeness in connection with the advertisement and promotion of the Event in any media or format.  Participant hereby waives any right to inspect or approve such materials.

This Waiver and Release shall be binding upon the heirs, next of kin, personal representatives, executors, and administrators of the undersigned.  In signing the foregoing release, the undersigned hereby acknowledges and represents that Participant has read the foregoing release, understands it, and signs it voluntarily and that Participant is of sound mind.
