Choir Uniform Purchase Form
In order to give a polished, professional appearance for performances, each student is required to choose and wear one of the two uniform options.  If any student is unable to provide the resources for a uniform, parents or guardians can discuss this in confidence with the director.  Financial accommodations may be arranged, based on student need.  Uniform Fees are due by August 31st to allow for alterations and finishing by the first concert.
  1. Uniform cost is $125 (one time fee) not covered by Student Ensemble Fees.
  2. All payments will be made through the OSP (Online Student Payment) portal.
  3. Students will not receive their uniform until it is paid in full or payment arrangements are made.
  4. This uniform will last for the duration of student's involvement with Johns Creek Chorus.
  5. Members of Johns Creek Chorus program will provide their own black, flat, closed toe shoes.
  6. Students will be responsible for maintenance and upkeep of their uniform.
