The purpose of this form is for parents to select an instructional program option for their child for the 2020-2021 school year. Based on the information provided in this survey, a placement and schedule will be created for your child. Please complete a separate survey for each child and submit by 1:00 pm on July 27th.

Ridgway parents should choose "Not Applicable". *
