2023 Youth/Teen Contest Registration

Please fill out this form to participate in the 3-Point Shoot Out AND/OR the Wiffle Ball Home Run Derby contests. Participants need to report to the contest location at least 10 minutes prior to the start of the event to check in with organizers. Family members, friends, and community members are encouraged to attend and cheer on participants. 
Parking is VERY limited. Please park near the Square with other Festival attendees and walk or ride the provided shuttle to the contest location.

I, the participant (if over 18 years old) OR the party responsible for the participant (if under 18 years old), understanding the following:
1. I am giving the individual listed in this registration form permission to participate in the contest(s) selected above. 
2. I understand the contest will be run by volunteers and supervision will NOT be provided before, during, or after the contests. Parents/guardians will remain responsible for their children and expected to supervise their children before, during, and after the contests. 
3. Contest facilitators will be treated with respect and kindness. Any negative verbal or physical interactions from participants or spectators will not be tolerated. Any such behavior is grounds for being disqualified and immediate removal from the contest areas.
4. Poor sportsmanship, disrespect, taunting, and rudeness from participants and spectators are all grounds for being disqualified and immediate removal from the contest areas.
5. Photos will be taken throughout the contest and may be used for marketing purposes on the Internet including social media. Additionally, the first name of winners will be shared on multiple channels unless contest facilitators are explicitly told not to do so.

By writing my name and relation to participant below, I am giving permission to the registrant to participant and agree to the stated expectations.

By writing below, I understand the risks involved and, in consideration for the opportunity to participate in the Four Seasons Festival youth/teen contests, do hereby release any and all rights and claims for damages now or hereafter have against the Four Seasons Festival, Polk City Chamber of Commerce, Polk City Development Corporation, the City of Polk City or their respective agents, representatives, successors or assigns, for any and all injuries which may be sustained by me in connection with my participation in the Four Seasons Festival Parade.
