By completing this form you agree to the following T&C's

Availability
All bookings are subject to availability. We cannot guarantee that any service will be available at any given time. In certain circumstances beyond our reasonable control, we may need to stop providing certain services. If this happens and it affects your booking, we will notify you by email, offer you an alternative service or cancel your original booking and provide you with a credit to use on your next booking.

Space is limited so therefore we can only offer spaces up to our allocated head count. Spaces are not confirmed until payment is made so to avoid disappointment, please ensure payment is made at the earliest convenience.

Payment SMMT Monthly -Saturday Morning Musical Theatre

SMMT costs £15 per week made payable monthly - (works out at £5 per hour - Bargain!)

NOTE- FOR JANUARY THERE ARE ONLY 3 SESSIONS THEREFORE ONLY £45 WILL BE DUE. 

Payment for your first month MUST be made within 3 days of completing this registration form to secure your childs place. Then payments must be made by the 1st class of the month for the full month’s sessions (Payment schedule will be emailed to you). We do not credit absences for pay monthly classes unless circumstances agreed. 

Late payment - a late payment fee of £10 will be due for payments made after this. Please be reminded that SMMT does not run during Easter and Christmas holidays but we do continue to run during the Feb, May and October half terms. We will be offering Summer workshops - bookable separately.

Payment Options - Classes can be paid via cash or Bank Transfer. We DO NOT accept card payments or Cheques.

Bank Details: Mrs Natasha Whitmore SC 60-11-15 ACC - 19729383 Reference Child’s name/surname. * without a payment reference we cannot credit your child’s name to payment. Which may result in a late payment charge if we cannot match your payment.

Payments are in full for the entire month child's place and are non-refundable.

Should your child no longer wish to attend it's ok we understand it may not be for everyone but we require one months notice via email. 

Upon registration - an email of the payment details will be sent to you, and upon confirmation of the first months payment, a confirmation email of your child's space will be provided.  Confirmation of your child's space is not granted until you receive this email. If you do not receive this email within 48hours of payment please contact us immediately at newheightsperformingarts@gmail.com

Timetable - A timetable will also be emailed upon registration. We do have 2 snack/ break times. So please pack some snacks and plenty to drink - yes they can have squash but NO FIZZY DRINKS. 

Absence If your child will be absent from any SMMT class due to illness, Please drop us a message out of courtesy. Also for our risk assessment/ evacuation reasons. If you child has been sick please remember the 48 hour rule applies - so if your child didn't attend school Friday, please do NOT send them into SMMT. We have some immune vulnerable students/ staff. 

Attire- Children must attend in appropriate clothing to dance. ie leggings, shorts, T shirts, hoody, and trainers. Hair MUST be tied back. Should a child not be wearing appropriate footwear they will not be able to dance. 
Children have the option to purchase an NHPA T Shirt these are £10 and ordered via emailing newheightsperformingarts@gmail.com or messaging into the whatsapp group.

First Aid
New Heights Performing Arts instructors are first aid trained however we will ONLY administer necessary first aid. Inhalers and epi pens MUST be provided with a diagnosis in this form so we can ensure your child gets the correct first aid. Aerosols are not permitted - ie body spray etc. 

NUT FREE ZONE - DUE TO ALLERGIES OF PUPILS WE ARE A NUT FREE SCHOOL.  

Fire evacuation
In the event of an evacuation we will follow the schools fire evacuation protocols

Photo Consent
By completing the appropriate section below you consent to your child's photo being used on both the school's social media platforms and for New Heights Performing Arts social media platforms and promotional material

Parents agree to refrain from taking photo's/videos unless otherwise explicitly instructed otherwise by the New Heights Performing Arts member of staff

Contact Consent
Your contact details will not be shared with 3rd party organisations with the exception of the school office.

If consent is obtained we will occasionally contact you about events and promotions provided entirely by New Heights Performing Arts

Show Commitment
On the occasion where we do annual shows, Students MUST attend all rehearsals. 

Commitment is essential for the success of any show, and we expect all students to prioritise rehearsals and performances accordingly. Each member of the cast and crew plays a vital role, and frequent absences disrupt the progress and cohesion of the production. Therefore, students must attend all scheduled rehearsals unless a prior arrangement has been made and approved by the principal. Persistent absences, or failure to uphold this commitment, may result in removal from the show. We value the effort and dedication of every participant and trust in your cooperation to bring our productions to life!


Respect
We agree to treat all parents and children with the utmost respect at all times during our sessions.

By signing this form you and your child are also committing to treating all parents, children and staff associated with any of our events with the utmost respect at all times. 

New Heights Performing Arts reserves the right to refuse any  parent or child  access to our classes following any threats, verbal or physical abuse or otherwise serious bad behaviour. Any incidents of this nature will not be tolerated and will be enforced through the full extent of the law if applicable. 

Welfare
The welfare of your child is of paramount importance and as a performing arts body we are obligated to inform the school and social services of any concerns we have.

Governance
New Heights Performing Arts holds public liability insurance and instructors also hold all relevant performing arts qualifications and enhanced DBS. All available on request.
