MontMilner Rainbow Camp FUNDRAISER Order Form

Please fill out a form for each sweatshirt/ long sleeve t shirt.

You may add any number of the holographic waterproof camp logo stickers to your order form.

Production will not start until payment is received.

Please choose a delivery method at the bottom of this form.

Options are: Mail, Grand Officer Practice, or Homecoming, Camp Day, Grand Assembly.

For mailing: Please allow up to 3 weeks for ordering, creating and shipping.

TOTE BAGS

Tote bag- 6.0 oz, 100% cotton with 9" handle drop (deminsions 10x12) $8

**Add traditional sprial tie-dye pattern to tote bags for an additional $2 per bag.**

STICKERS

2.5 “ Montmilner Rainbow Camp holographic, glossy, waterproof sticker. $3 each

Once your order has been received, a total invoice will be sent to you including payment processing fees. Payment must be received before production will start on your item(s).

If you choose mail delivery, your invoice will also include the shipping cost.

Please allow 1-2 days for us to respond to your order.
