Complete this form for each child attending camp.  Once you have completed the form, you will receive a code to use on Camp Lebanon's website when registering!  You will have the option to enter a 'promotional code' when you start your check out on Camp's website.  Be sure to SUBMIT your form at the end!

This is available for all youth from Faithbridge who are attending Camp Lebanon, regardless of financial status!

Parents are responsible for providing rides for their children to and from Camp.  Check the Camp Board in the West Foyer to see who is going the same week for carpool options.  If a group of campers is too large to fit in a personal vehicle, call the Church Office (218-732-1404) to check the availability of a church van and/or driver. The Church Office must be notified 1 week prior to the start of camp week.
