Spotter Registration Form
Thank you for volunteering to be a part of the NWS Melbourne SKYWARN Spotter Network. The reports that SKYWARN spotters provide are one of the most important assets to the warning, forecast, and verification process during a severe weather event. 

This form is for residents, at least 18 years old, of east-central Florida, i.e. Brevard, Indian River, Lake, Martin, Okeechobee, Orange, Osceola, Seminole, St. Lucie, or Volusia County. Residents younger than 18 can take the class for educational purposes. You will be issued a Spotter ID number and card upon completion of the class. Please submit one registration form per person.
